Following revisions to the Authorised Processes list in March 2024, clinics are now required to submit a completed copy of the Proposed Licence Activities and Authorised Processes application form alongside the following online licence applications:

When making an application, the Person Responsible should first submit a completed licence application via the Clinic Portal. This form must then be returned to compliance@hfea.gov.uk, with the application reference quoted in the subject.
Failure to submit this form may delay the consideration of your application.
