Consent forms

All forms below use gender neutral language except the SPP, LC, SWP and WP forms. Versions of these forms using gender neutral language can be found at the bottom of the page.

Guidance

This guidance sets out how each HFEA consent form and statutory notice should be used. The guidance below should be read in conjunction with the Code of Practice.

Record of information provided before obtaining consent

We recommend clinics use these forms to record any information given to patients, donors or surrogates around consent.

Treatment and storage

Consent forms for patients having IVF, ICSI, IUI and GIFT as well as those storing their eggs, sperm or embryos.

Renewal of consent to storage

Forms for patients wishing to renew consent to the storage of their eggs, sperm or embryos.

Statutory notices

Statutory notices that clinics are required to issue to patients regarding the storage of their gametes or embryos at defined points during their storage period.

Donation

Consent forms for people donating their embryos, sperm or eggs, including egg sharers.

Surrogacy

Consent forms for surrogates and intended parents in surrogacy arrangements.

Only to be completed if conditions are met under the 2024 Regulations

This form must only be used for a small cohort of patients who are creating and storing embryos created with the gametes of their deceased partner, where those gametes fall under The Health and Care Act 2022 (Storage of Gametes and Embryos) (Transitional Provision) Regulations 2024. Before using this form, clinics must ensure that the gametes meet the conditions set out in law. Training in the event of mental incapacity

Consent form for patients who wish for their eggs, sperm or embryos to be used in training were they to lose capacity.

Withdrawal

Consent forms for people withdrawing consent to the use or storage of their eggs, sperm or embryos, legal parenthood, or surrogacy.

Legal parenthood

People having treatment with donor sperm (or embryos created with donor sperm) and who are not married or in a civil partnership must complete the below legal parenthood forms to ensure they/their partner are recognised as a legal parent.

Mitochondrial donation

Only clinics licensed to carry out mitochondrial donation can use these forms. The ‘MD – including PNT’ form can only be used by clinics supplying sperm to clinics licensed to carry out mitochondrial donation.
